Daytona Beach, FL - The Florida Highway Patrol says no seat belts or child restraints were used in a rollover accident where a mother and 2 of her children died on I-95 southbound in Daytona Beach.

Troopers report that the wreck happened 3:25pm Friday at LPGA Boulevard, near Mile Marker 265.

Witnesses state that the 2010 dark-colored Mercedes van – which FHP says was driven by 38-year-old Michelly Zulfiqar – was heading southbound on I-95 near Mile Marker 265 when the vehicle unexpectedly went off the road onto the shoulder and struck a wall on the LPGA overpass that crosses I-95.

Those same witnesses tell FHP the van apparently hit the wall without the driver hitting the brakes first, causing it to roll over.

FHP Sergeant Kim Montes says Zulfiqar and her 10-year-old son Hami were declared dead at the scene by first responders.

The other 2 in the van – Zulfiqar’s 3-year-old daughter Dani and 4-year-old son Sami – were freed from the trapped vehicle and rushed to Halifax Medical Center, but Montes said the girl passed away while at the hospital.

The 4-year-old boy was reported initially in critical condition, but is now stabilized and at Arnold Palmer Hospital along with his father, according to Montes.
